Medical Release Form:
1. Go to the Crossings homepage: www.crossingscamps.org
2. Look to the bottom left of the page and click on the red “Medical Release & POI Forms” box.
3. You will be directed to the Medical Release & POI Forms Login page. Click on the “Sign up” button in the “New Participants” box on the right of the page and follow the prompted directions.
4. Complete the form, submit, and print out.
POI Form:

1. Go to the Crossings homepage: www.crossingscamps.org
2. On the top right, click the “At Camp” button.
3. Select the camp location.
4. See information, pictures, and video about a specific camp week, meet the camp staff, email your child at camp, and more.
